- Type of magnifier or telescope selected.
- Magnification level selected.
- Other notes.
- Referral to eye health professional.
- Follow up date.
Question
Why is it important to write down this information?
- The information may be referred to in the future when the person has a follow up appointment. It will help the service provider to understand what the person has previously received.
- It also assists the service provider with stock management, and service reporting.
