Plan

Lesson: 2 of 6
Topic: 6 of 7
0% Complete

After the assessment interview, you will need to make a plan.

Screenshot of the assessment form showing the section on planning.

The plan might include:

  • Selecting a new walking aid with the person
  • Making a referral to other services
  • Deciding with the person if the current walking aid meets their needs and if any adjustments or repairs are needed
  • Deciding with the person what training is needed to use the walking aid safely
  • Making a follow up appointment.

Remember that selecting a walking aid should always be done with the person.

In the next topic, you will have a chance to practise using the assessment information to select the best walking aid.

When to make a referral

If you think the person needs to see another service provider, you should always make a referral.

Question

When do you think you might need to make a referral?

  • When a person needs rehabilitation
  • When a person has a new health problem
  • When a person has an open wound that is not being treated
  • When a person might be at risk of a foot wound
  • When a person might need another assistive product that you cannot provide.

There are many reasons – you may think of some different ones.
