Participant information sheet
Welcome to Training in Assistive Products (TAP). This training is the first step of the project, “Training in Assistive Products (TAP)”, led by the Ministry of Health of Tanzania, the World Health Organization and HelpAge International Tanzania. This project is intended to establish a community level referral system for assistive technology, and provision of a selected range of simple assistive products at primary health centres and two district hospitals.
Please read the information below and complete parts A (agreement) and part B (enrolment survey) before you begin the training
Information about TAP: TAP is an online learning programme for personnel who are, or will be:

The TAP modules you are taking can be accessed using a computer, tablet or smart-phone. During your TAP learning, you will also have face to face support from mentors.
If you have a question now, or at any time when you are going through this training, you can:
- Обговорити питання зі своїм наставником, координатором проекту
- Discuss with your colleagues
- Post your question on the communication group your mentor will set up (check with your mentor)
Your feedback is important: At the end of the training, you will be asked to complete a short (20 minutes) online feedback survey. You may also be asked to provide feedback in other ways such as through a group discussion (of up to 90 minutes). Your participation in these feedback activities is voluntary, and they will be carried out during working hours, at a convenient time for yourself and your service manager / supervisor.

Також збираються бали вікторини та інформація щодо кількості та типу модулів, які ви опрацьовуєте. Коли слухачі беруть участь в дискусійних групах для обміну досвідом і надання зворотного зв'язку, може бути зроблений аудіозапис обговорень, а потім використаний для створення письмового запису. Після цього аудіозапис буде видалено.
Before this information is used it will be de-identified. This means that the names, and personal details are removed. This way, no-one looking at the information will know whose information it is.
The de-identified information is then used to help prepare reports about this TAP training and for research to help understand:
- How well TAP is working for learners and how it can be improved
- The thoughts of learners about the provision of assistive products in their workplace
- What other actions could be taken to improve access to assistive technology.
The de-identified information is held securely by the World Health Organization. It may be combined with data from other TAP projects, and it may be shared with project partners, donors, and researchers.
