Plan actions needed

Use the information collected to see whether:

• The therapeutic footwear needs repairs or replacement.
• The person needs a different product. If yes, can the product be provided locally or is a referral needed?
• The person needs more training about how to use their therapeutic footwear.

Plan next follow up

Plan the next follow up with the person.

They must attend at least two follow ups after first being provided their therapeutic footwear. Afterwards, a review each year is needed.

Provide the next follow-up date and time to the person on a written card to ensure they remember to attend.
