Assessment includes:
- Assessment interview: asking questions of the person and sometimes also their support person or family member
- Mobility assessment: watching how the person moves around
The walking aid assessment form used in this module is a simple tool that can be used as a guide during assessment. The form includes questions to ask and what to look for in the mobility assessment. The form also has some space to add comments and includes some tips to help guide selection.
If you haven’t already, download the assessment form and print a copy.
If you are unable to download the form, don’t worry. Each of the questions on the form are clearly shown as you go through this section of the module.
