Step Four – Follow up
The fourth step is to follow up. This important step is an opportunity to check whether the assistive product is still meeting the person’s needs and in good condition.
Question
- What are some actions a service provider may need to take during a follow up?
- Check that the assistive product is still meeting the persons needs
- Check the assistive product is in good working order
- Answer questions about the assistive product
- Adjust the fit of the assistive product
- Carry out or arrange for repairs or maintenance
- Give further training in how to use or look after the assistive product
- Refer the person to other services if needed
Remember Mathias?
His community worker visits him every six months to check that his rollator and other assistive products are still meeting his needs.
He checks the rollator to make sure it is working well and is safe, and asks Mathias and his wife if they have any questions or difficulties.
During the last visit, the community worker fixed the rollator brakes. He also referred Mathias back to the nurse for review, as Mathias’ therapeutic footwear had become uncomfortable.
Following the four steps will support service providers to safely and effectively provide assistive products so that they best meet each person’s needs.
